ACCORD CARE is a trusted provider of person-centered disability support services to NDIS participants. We deliver Supported Independent Living (SIL), in-home care, community participation, short-term accommodation, and community nursing.

About the Role
We are seeking dedicated Support Workers to join our Supported Independent Living team in Norton Sands. In this role, you will support participants to live as independently as possible while engaging actively in their community.

Key Responsibilities

  • Assist participants with personal care and daily living activities, including meal preparation, household tasks, and medication reminders
  • Support participants to access the community, attend appointments, and engage in social and recreational activities
  • Follow individualised Behaviour Support Plans and Risk Assessments to ensure safe, high-quality care
  • Maintain accurate documentation, including shift notes, incident reports, and data collection
  • Collaborate with team leaders to implement and review participant support plans
  • Maintain professional relationships in line with NDIS and organisational standards
  • Contribute to positive participant outcomes and team objectives
  • Be reliable, accountable, and person-centered in all aspects of care

Qualifications & Experience

  • Certificate III in Individual Support, Aged Care, or higher (or willingness to obtain)
  • Experience supporting participants with complex diabetes
  • Competent in BGL monitoring and recording
  • Ability to administer insulin (pen or syringe) safely
  • Experience testing and recording ketone levels
  • Ability to follow diabetes care plans and protocols
  • Experience in mental health and challenging behaviours
  • Nursing experience highly regarded
