This is a high-responsibility Supported Independent Living (SIL) role supporting a participant with complex physical care needs in Ferntree Gully.

This is a high-responsibility Supported Independent Living (SIL) role supporting a participant with complex physical care needs in Ferntree Gully.
You will work as part of a consistent, trained support team delivering structured, hands-on care where safety, accuracy, and calm decision-making are essential every shift.

What Your Shift Will Involve
You will be responsible for delivering safe, structured care including:
- Providing personal care including hygiene, grooming, and daily living support
- Performing safe manual handling and hoist transfers (2:1 support where required)
- Supporting mobility using assistive equipment such as wheelchairs, pressure care mattresses, and adjustable beds
- Assisting with meal preparation including modified diets and dysphagia-aware support
- Providing continence care and supporting skin integrity management
- Completing pressure care routines and scheduled repositioning
- Monitoring participant wellbeing and escalating any changes immediately
- Supporting household tasks including cleaning and laundry
- Following detailed care plans, risk management strategies, and escalation procedures at all times
- Completing accurate documentation during and after shifts

- Identifying and escalating changes in condition early and accurately
- Working effectively within a structured SIL team environment
- Strict adherence to care plans, clinical guidance, and safety protocols
- Calm, reliable, and confident in high-responsibility care environments
- Experienced in high physical support, complex care, or SIL settings
- Comfortable following structured clinical care plans and escalation pathways
- Strong in communication and teamwork within care environments
- Committed to maintaining dignity, safety, and consistency in care delivery
- Prefer low-responsibility or routine-only care work
- Are not confident with manual handling or complex care requirements
- Struggle to follow structured procedures and clinical direction consistently
- Certificate III or IV in Disability, Individual Support, Aged Care, or equivalent
- Experience in high physical support or complex care environments
- Manual handling and hoist experience
- Experience with personal care, continence care, and meal assistance
- Strong understanding of care plans, scope of practice, and escalation procedures
- Current NDIS Worker Screening Check (or willingness to obtain)
- Current First Aid & CPR (or willingness to obtain)
- Full Australian working rights
- Experience in Supported Independent Living (SIL) settings
- Knowledge of dysphagia and swallowing risk management
- Nursing or clinical background
