Disability Support Worker
ACCORD CARE is trusted in the provision of person-centered disability support services to NDIS participants. We provide Supported Independent Living, In-home care, Community Participation, Short-Term Accommodation, Community Nursing, and Allied Health Services.
About the role
We are looking for Disability Support Workers and Team Leaders in our supported independent living arrangements across multiple locations in suburbs of Melbourne like Frankston and surroundings to provide 24/7 support in the home and out in the community.
The support worker's role and responsibilities will include:
- Supporting people with personal care and day-to-day activities in the home and out in the community
- Providing positive behaviour support to our clients
- Completing appropriate documentation through shift notes, incident reporting and data collection
- Contribute to the team’s overall objective of providing the best outcomes for our clients
- Work with your team leader to implement and review plans around daily service provision
- Maintain a professional relationship with clients and adhere to the NDIS and company code of conduct
- Above all, be responsive, reliable and accountable to our clients and your team

Part-time/Casual positions
Competitive Pay Rates up to $44.39/hour
Weekend Rate: up to $79.90

Qualification/Experience:
Minimum Certificate III in Individual Support/Aged Care or Higher
(or the ability to acquire an appropriate qualification)
Mental Health Experience
Problem-Solving Skills
Challenging Behavior Experience
Verbal/Written Communication and Relationship Building Skills
