About the Role
We are seeking an experienced and compassionate Registered Nurse – Care Coordinator to join our team on a casual basis (3 days per week, 8-hour shifts). In this role, you will provide direct nursing support to participants while ensuring their health, safety, and well-being. You will also play a key role in training and supporting staff to deliver high-quality, individualised care.

About the Role

In this role, you will provide direct nursing support to participants while ensuring their health, safety, and well-being. You will also play a key role in training and supporting staff to deliver high-quality, individualised care.
Key Responsibilities
- Deliver person-centered nursing care to NDIS participants
- Develop, implement, and review individualised care plans
- Provide clinical guidance, supervision, and training to support workers
- Conduct health assessments and ongoing monitoring
- Support behaviour management strategies, including restrictive practices
- Collaborate with families, carers, and multidisciplinary teams
- Maintain accurate documentation and ensure NDIS compliance
- Respond to complex care needs and crisis situations
- Strong client-focused approach with excellent communication skills
- Ability to adapt communication style to suit different audiences
- Sound computer literacy (Microsoft Office, email systems)
- High level of confidentiality and professionalism
- Minimum 2 years 'experience in disability, mental health and complex care
- Minimum 2 years' experience in community nursing (preferred)
- Understanding of restrictive practices and behaviour support
- Experience supporting individuals with challenging behaviours
- Experience in Supported Independent Living (SIL)
- Experience supporting people living with dementia
- Experience in training and developing staff
- Current AHPRA registration as a Registered Nurse
- Experience in disability, community nursing, or allied health settings
- Ability to lead care teams and manage conflict
- Strong skills in care planning, report writing, and compliance
- Ability to work autonomously and within a multidisciplinary team
- Excellent time management and organisational skills
- Current Driver’s Licence
- NDIS Worker Screening Clearance
- Working with Children Check (Blue Card)
